The mother of a 23-year-old man who took his own life more than a year ago has spoken of her anger over delays to an inquest into his death.

Tyrone Ward, known as Ty, was found dead in Mousehold Heath in April last year, having suffered with mental ill health.

An inquest into his death was opened shortly after the tragic incident, after which his mother paid tribute to her son.

But a full hearing is yet to be held, with Norfolk Coroner's Court still waiting for the Norfolk and Suffolk NHS Foundation Trust to submit evidence about the care he received.

The delays have sparked anger from Maria Ward, Tyrone's mother, who accused trust bosses of using stalling tactics to minimise scrutiny on its services.

Mrs Ward, from Norwich, said: "The delays are having a massive impact on my mental health.

"I desperately want answers about what happened with Ty and just need to know when I'm going to be able to get that closure.

"I fully believe his death could have been prevented.

At a brief hearing last week, area coroner Yvonne Blake confirmed the case was not yet ready for inquest due to outstanding reports from the trust.

"I relive what happened to Ty every day - but I just need answers about it and don't feel I am getting them from NSFT," Mrs Ward added.

Anthony Deery, the trust's chief nurse, said: "We would like to offer our sincere condolences to Tyrone's family for their tragic loss and apologise for the time it has taken to prepare reports for the coroner.

"We have carried out an extensive investigation into the care he received so we can make sure the coroner has all the information they need to establish the facts around his death and answer any questions his family may have.

"This patient safety incident report is now complete and is in the final quality checking stages before publication.

"We have been working with Tyrone's family and keeping them regularly updated."
